Incident/Accident Reporting

If you have been involved in an incident/accident upon the NCA Estate, please click here to complete the form.

Please include as much detail as possible including name, address, incident description, location of incident (drop down menu) and witness details.

Upon submission (save) of the form, you will be provided with an incident reference number, which will be utilised by the WHS team for responses and management of the incident.

Should you require an update on the status of your incident, please email whs@nca.gov.au using your incident reference number in the subject line.
